Choose From Two Convenient Payment Options

Assurant Health
makes paying for your policy easy by offering two convenient payment
options and three payment methods (credit card, bank account debit or check).
Single
payment option:
ideal if you know the exact number of days coverage is needed. The
minimum number of days you can purchase is 30 and the maximum is 365.
Your entire premium can be paid by MasterCard, VISA or
check.
Monthly payment
option: ideal if you are unsure how long you need coverage. This "pay as
you go" option gives you the flexibility to continue coverage for as
long as it's needed (up to 365 days) or simply stop payments and
discontinue the plan whenever you want. You make an initial premium
payment for 35 days.

If you
pay your initial premium by
MasterCard, VISA
or bank account debit,
each additional 30 days of coverage will be automatically charged
to your credit card or debited from your bank account for up to 365 days of coverage. If your
temporary need ends prior to the 365th day, simply call
1-800-800-5453 and Assurant Health will stop the automatic credit card
or bank account
debit.
(Please note: 7 days advance notice is required to
ensure future debits are stopped.)

If you
pay your initial premium by
check,
you will receive payment coupons shortly
after you receive your contract. Each coupon is for an additional
30 days of coverage. Each month, mail a check along with the
coupon to Assurant Health. (Please note: no lapse notices are
sent.)

Assurant Health is confident that their
Short Term Medical plan will satisfy your temporary health insurance
needs. If
you are not completely satisfied with the plan, you can return it within 10 days of issuance for a
premium refund. No questions
asked! Where the one-time application and administration fee apply,
the fees are non-refundable. Simply put your request in writing and fax it to 414-299-6217

When Does Your Coverage Begin? |

- Internet using a credit card
The earliest your coverage can begin is the day following
transmission, if all other eligibility criteria have been met. For
example, if you submit your application on-line March 16th, your
coverage begins at 12:01 AM on March 17th.
All transmissions take place and are recorded based on the time and
date in the Central Time Zone. For example, if you submit your
application on-line at or after 10 PM on March 15th from Washington
State, the time of the transmission will be at or after 12 AM
Central Time. The transmission date of your application will be
March 16th.

Mail and writing a check
The earliest that your coverage can begin is
the day following the U.S. Postal Service postmark, if all other
eligibility criteria have been met. (If the envelope containing your
application is not postmarked by the U.S. Post Office or if the
postmark is not legible, the plan date will be the later of a) your
requested date; or b) two days prior to the date the application was
received by Assurant Health.)
Coverage will take effect provided the following conditions are met:

completed application and the full premium payment are received by
Assurant
Health; and

Your
answers on the application are complete and meet the requirements
for acceptance.
