LifeWise Health Plan offers 3 plans with different coverage levels and prices from catastrophic and moderate to enriched and tax-advantaged options available to people living in all areas
of Washington State.

All plans are Preferred Provider (PPO) plans, so no Primary Care Physician (PCP) is needed and no referrals are required. Click here for the provider directory.

  • WiseChoices 20
  • WiseChoices 30
    • Annual deductible: $1,000 (WiseChoices 20) or $1,500 (WiseChoices 30)
    • Office visits: no deductible for office visits
    • Network benefit: 80% (WiseChoices 20) or 70% (WiseChoices 30)
    • Annual coinsurance maximum: $8,500
    • Preventive care: covered (no deductible)
    • Prescription drugs: covered to $3,000 per year for brand name drugs;
      no limit for generics
    • Maternity: covered
    • Vision exam, lenses and frames: covered (no deductible)
 
WiseEssentials
  • WiseEssentials $1,750
  • WiseEssentials $2,500
  • WiseEssentials $3,500
    • Annual deductible: $1,750, $2,500 or $3,500
    • Office visits: no deductible for first 6 visits per calendar year
    • Outpatient diagnostic x-ray & lab: no deductible (WiseEssentials $1,750 only)
    • Network benefit: 75%
    • Annual coinsurance maximum: $5,000
    • Preventive care: covered
    • Not covered: prescription drugs, maternity, durable medical equipment, prosthetics, vision exam, lenses and frames
 
WiseSavings (HSA-Qualified)
  • WiseSavings $1,750 / $3,500
  • WiseSavings $3,500 / $6,000
    • Annual deductible:
      Individuals - $1,750 or $3,000; Families - $3,500 or $6,000
    • Network benefit: 80%
    • Annual coinsurance maximum:
      Individuals - $2,500 or $1,750; Families - $5,000 or $3,500
    • Preventive care: covered (no deductible)
    • Not covered: prescription drugs, maternity, vision exam, lenses and frames
